Candidate's Bio-Data form and Attestation Form for SBI Group Discussion and Interview.

This form is sent to all those who are called for the Group Discussion and Interview in the SBI PO and its Associates Banks PO exams. I believe the same form is applicable to Clerks too.

You will have to fill in the details and carry it to the GD/PI venue.
